This leads to individual’s feeling ashamed of not being able to understand the information and becoming afraid to ask questions. To reduce the negative effects of limited health literacy, our institution has instituted the technique of, “Teach Back” for all patient education sessions. Teach Back allows us to educate the patient or patient’s caregiver and validate their understanding in a non-threatening way. Teach Back is a simple concept. In a non-threatening way, after any teaching session, ask the individual to explain in their words what they understood. If there is a gap in patient understanding, the health care provider can review the information again. For our Heart Failure patient population, we simplified and standardized our education. We developed a list of four questions that are vital for the patient or caregiver to know in order to provide a safe discharge home. This allows us to limit the amount of information the patient is receiving as “need to know” versus “nice to know,” We have them repeat the information back, in a non-threatening way, to assure understanding and comprehension of the information provided. These four questions are located on the first page or our CHF patient education packet. I It is visible for staff to review these questions during routine patient interaction. Examples of Teach Back include: When a nurse passes her medication- she can ask, “What is the name of your water pill?” Or, During a teaching session-she can say, “I teach people about this everyday. Sometimes I may not make myself clear. I want to make sure you know what you need to do. So, can you tell me what signs and symptoms of heart failure you are going to call your doctor about?” This process allows for patient validation of their understanding in a non-threatening way.
